Procedures at PRHC (endoscopy, surgical procedures)
If your operation or scope was scheduled PRIOR to March 16, to be PERFORMED after this date, you WILL NOT proceed as planned. You will be re-booked when possible. Cases will be re-booked based on urgency. Please let us know if there is any change in your condition.

Visits to the Peterborough Clinic
Your visit will proceed in person ONLY if necessary. You will be required to answer screening questions and REQUIRED to wear a mask. To expedite your entry into building you can print and fill in this form or answer the questions on your mobile phone using this form. Most visits will be performed virtually, by telephone or using secure video conferencing such as Medeo, or the Ontario Telehealth Network OTN eVisit. Please let us know of your preferences prior to your visit. If you are interested in a video virtual visit, please the links on this page.
Follow up care

Please obtain a referral from your family doctor or nurse practitioner to schedule an initial consultation.
If a referral has already been sent, you will be contacted directly by mail or phone (if you have voicemail) with a date and time. Please bring a list of your current medications.
Appointments are booked on the basis of urgency. If your condition becomes more urgent, please have your family doctor or nurse practitioner make us aware of the urgency of changes in your condition. For routine consultations there may be a wait of a few months.
Taking Care of You
To book a post operative or follow up appointment, please contact our office. Call our office the week following your procedure to book an appointment in the suggested timeframe if this has not already been done. Leave a message including your name and phone number so we can get back to you.
The Care You Deserve
Please make us aware prior to your visit of any medications you are taking that may worsen bleeding. This includes ASA, Plavix, coumadin, and DOACs/NOACs. Wear comfortable clothing and appropriate footwear.
Please note that Dr. Ebisuzaki does not perform cosmetic procedures.
Some follow up appointments can be arranged online using Medeo, an online appointment booking service, free of charge. You will need an email address to use this service. Please contact our office and leave a message if you are interested in online booking.
